2. Swan Neck Deformity

  • Definition: A deformity characterized by hyperextension of the PIP joint and flexion of the DIP joint.
  • Etiology: Secondary to mallet injury, FDS rupture, or intrinsic contracture. It may also result from MCP joint volar subluxation.
  • Pathology: Imbalance of forces across the PIP joint, where PIP extension forces exceed flexion forces.
  • Clinical Presentation: PIP hyperextension with DIP flexion, often seen in rheumatoid arthritis or following tendon injury.
  • Treatment:
    • Conservative: Extension splinting of the DIP joint for 6-8 weeks.
    • Surgical: Central slip tenotomy (e.g., Fowler procedure), FDS tenodesis, or oblique retinacular ligament reconstruction. Mcq ❓️
