Definition: A deformity involving flexion of the PIP joint and extension of the DIP joint, often due to rupture or injury of the central slip of the extensor tendon.
Etiology: Trauma, laceration, capsular distension (e.g., rheumatoid arthritis), traumatic avulsion, and rupture of the central slip.
Pathology: Rupture of the central slip, attenuation of the triangular ligament, and palmar migration of collateral bands. Mcq ❓️
Clinical Presentation: PIP joint flexion, DIP extension, and loss of active PIP extension. The Elson test can help diagnose it.
Imaging: X-ray (AP and lateral view) of the affected hand to assess joint alignment.
Treatment:
Conservative: Splinting of the PIP joint in full extension for 6 weeks. Double ring splint may be used for acute injuries. Mcq ❓️
